You have to fill out the medical self-declaration that you can download here. If you answer NO to everything in the first box, you just need to sign the declaration and that's it. If there is any question, the answer is YES, they make you fill in other boxes (A, B, C, D, E, F, G...).
If you answer NO to the questions in boxes A, B, C, D, E, F, G, it is not necessary to do an official medical check-up. If you answer YES in these boxes, you have to go to your GP or any other doctor so that they can fill out the doctor's evaluation form.
